Impostor syndrome refers to persistent beliefs of fraudulence and inability despite objective successes. It is linked to patterns of brain activity in self-evaluation, error monitoring and reward systems, as well as cognitive biases.
The anterior cingulate cortex (ACC) monitors conflict between internal self-image ("I am not good enough") and external feedback (praise, promotions). In impostor experiences, ACC may be hyperactive, maintaining focus on perceived errors and discrepancies.
This supports a negative attention bias: successes are discounted, while mistakes are magnified.
The ventral striatum and dopaminergic circuits normally encode success as rewarding and attribute achievements partly to one’s own abilities. In impostor syndrome:

The orbitofrontal cortex (OFC) evaluates outcomes relative to expectations. Unrealistically high standards create a constant sense of falling short, even when results are objectively adequate or excellent.
This reinforces the narrative "if they really knew how it is inside, they would not approve", sustaining impostor feelings.
The medial prefrontal cortex and ACC are engaged during social comparison. Persistent upward comparison (to more advanced peers, curated images in social media) biases evaluation towards perceived deficiency, regardless of absolute performance.
Impostor syndrome thus represents a systematic distortion in self-evaluation networks rather than a reflection of true incompetence. Targeted cognitive and behavioural work can recalibrate these networks over time.
Impostor syndrome involves chronic self-doubt and fear of being exposed as a fraud despite objective evidence of competence and success. Neurobiologically, it stems from dysregulation in neural circuits that evaluate self-competence and internalize achievements. The brain's threat detection system (amygdala) becomes hyperactive around evaluation and performance, while the reward system fails to properly encode accomplishments. There's a disconnect between actual performance and internal self-assessment, mediated by cognitive biases like discounting positives, attributing success to luck, and overweighting criticism. The prefrontal cortex's rational evaluation is overwhelmed by emotionally charged negative self-beliefs formed earlier in life. High achievers may have particularly strong impostor feelings because achievement contexts trigger heightened self-monitoring and comparison. The "organism as a team" approach helps by recognizing that your worth isn't contingent on performance, your team (trillions of cells) has been working successfully to keep you alive and functioning every single day. Achievements are collaborative efforts between your abilities and circumstances.
