Agoraphobia is characterized by marked fear or anxiety about situations where escape might be difficult or help unavailable in the event of panic-like symptoms or other incapacitating events. It often co-occurs with panic disorder but can also present independently.
Individuals typically fear and avoid two or more of the following:
Fear relates to the consequences of anxiety symptoms (fainting, losing control, embarrassing oneself) rather than to the physical danger of the place itself.
Neural circuits overlap with other anxiety disorders: heightened responsivity in amygdala and insula, altered prefrontal regulation.
Collaborative planning and paced exposure are critical to avoid overwhelming the patient and to support durable change.
Agoraphobia demonstrates how fear of fear itself can restructure a person’s geography. Systematic exposure and cognitive work help rebuild access to previously avoided environments.
